If you wish to contact the appropriate licensing or disciplinary authority for our facility or providers, you may reach the following entities:

Filing a Consumer Complaint:
If you believe your rights regarding access to your health records have not been respected, you may file a complaint:

  • With Us: Please email farren@flourishwithfarren.com. 

  • With the State: Complaints may be filed with the Texas Attorney General’s Office and/or the relevant licensing authority. For information on filing with the Attorney General’s consumer protection division, visit https://www.texasattorneygeneral.gov or call (512) 463-2100.

  • With the Texas Health and Human Services Health Care Regulation

  • Clients may also file a consumer complaint with the Texas Health and Human Services Health Care Regulation: https://www.hhs.texas.gov
